How it wORKS

Prior to your appointment you will fill out an extensive health history form. It is also advised to share any medical records or labs from other providers. During your 60-90 minute in person or virtual appointment, we will review all medical records and intake forms, discuss your current health challenges, lifestyle and daily routines, and clarify your health & lifestyle goals moving forward. The rest of the appointment will be tailored to meet your individual needs. If desired, this may include a comprehensive physical and/or gynecological exam for in person meetings. We will explore tools such as targeted labs, professional supplement suggestions, nutrition support, body care, and mindfulness practices. The goal is to leave this appointment with steps for optimizing your health, daily routines, and nutrition through nervous system and blood glucose regulation.

For established NaturaVita clients- 30 & 60 minute follow up appointments are offered for reviewing blood work results, specialized test/lab results and going over healing protocols. This is also the time for additional support and adjustments to protocols when needed and adjusting supplements. Committing to root cause health solutions, and lifestyle changes, takes patience and dedication, and will often require more than one initial appointment.

I understand that navigating the complexities of the healthcare system can be challenging, and I strive to offer a different approach at NaturaVita. My primary goal is to prioritize the well-being of my clients and provide them with the best possible care without being limited by insurance restrictions. Regrettably, insurance providers do not offer reimbursement for the extensive evaluation required to thoroughly examine your personal history, health, and laboratory results. Additionally, they do not extend coverage to services that primarily focus on individualized preventive care. I value transparency and strive to maintain affordable and upfront pricing for my services with payment expected at the time of each visit.

Yearly comprehensive blood panels run through LabCorp are covered by Medical and are often covered by PPO insurance plans minus your co-payment (if applicable). Sometimes it can be less expensive to pay out of pocket though. I advise calling your insurance provider prior to our appointment and ask what they will cover and what you will be responsible for.
Specialty labs are often used as a tool to uncover imbalances and get a comprehensive look at how your body is functioning. These tests are not covered by insurance.

Some examples of the most common specialty labs are:
Hormone Saliva Profile III- $170- This ZRT salivary hormone profile tests the sex hormones and the adrenal hormones. All saliva profiles test waking levels of estradiol, progesterone, testosterone, DHEA-S, and cortisol. The three saliva profiles are distinguished by the number of times per day cortisol is tested. Profile III tests a full diurnal cortisol profile at four-time points during the day (morning, noon, evening, and night). This test is for all genders.
Full Immunology Profile- $448.33-
The Full Immunology Panel assesses food and inhalant allergies and sensitivities by combining the following panels: IgE Food Antibodies, IgE Inhalants, IgE Molds, IgG Food Antibodies, IgG Spices, IgG Vegetarian, and Celiac Profile.Environmental Toxins- $270- The Environmental Toxins profile from Vibrant Wellness assesses 39 different toxicants to determine the presence of excessive environmental toxin exposure.
NeuroAdvanced Profile- $212- This neurotransmitter test by ZRT assesses levels of specific neurotransmitters, amino acid precursors, and metabolites. Testing a full panel of markers provides a framework for understanding the connection of our body's physical health to our brain's well-being. This is an excellent test for people with anxiety, depression, OCD, ADHD, PPMD and more.
GI-MAP- $354.38- The GI Map from Diagnostic Solutions Laboratory is a comprehensive stool analysis that measures gastrointestinal microbiota DNA from a single stool sample. Using state-of-the-art, quantitative polymerase chain reaction (qPCR) technology, the GI-MAP evaluates gut pathogens, bacterial infections, yeast overgrowth, viruses, and parasites. Antibiotic resistance genes and virulence factors are also included on the test for unparalleled insight into H. pylori.

The Hormone and Urinary Metabolites Assessment Profile (HuMap™) provides a comprehensive overview of steroid hormones, their metabolites, and the efficiency of the enzymes that metabolize these hormones.Organic Acids Profile (OATS)-$310.30 A comprehensive metabolic analysis of a patient's overall health, including intestinal yeast and bacteria, vitamin and mineral levels, oxidative stress, neurotransmitter levels, and oxalates.

Both in-person and virtual appointments are options for your care. The only difference in services is a comprehensive physical exam and/or gynecological exam is an option for in-person appointments. Often follow up appointments are virtual regardless of an initial appointment being in-person.

I am not a primary care provider. Therefore, I recommend that all clients have a designated primary care provider (PCP) to address any urgent medical concerns, as I do not provide on-call services, and communication from the office might take up to 48 hours to receive a response.

Currently, there is a growing emphasis on safeguarding patient privacy and protected health information (PHI) through the implementation of various laws. As a result, all scheduling, payments, medical records, educational handouts and communication lines and messaging services, are carefully managed to adhere to the standards outlined in the Health Insurance Portability and Accountability Act (HIPAA) by using Jane EMR and email.

I work with clients of all ages and genders. For women and individuals across the reproductive lifespan, I provide comprehensive integrative wellness care within my scope as a Licensed Midwife and Certified Integrative Practitioner. For men, I provide wellness education, functional lab interpretation, and supplement and lifestyle protocols. NaturaVita Wellness is a safe and inclusive space for all individuals regardless of how they identify.

Yes. NaturaVita Wellness offers hormone therapy prescribing directly within the practice under a formal Collaborative Practice Agreement with Dr. Timothy Paik-Nicely, MD, a board-certified physician with nearly 50 years of experience in family medicine and women’s health in Humboldt County. Hormone therapy is available for clients of any age, gender, or stage of life. This service is currently available to California clients only.

Not always. If hormone therapy comes up during your regular wellness visit and you decide together to move forward, the prescription can be initiated from that appointment with a one-time $95 add-on fee. If you need a refill, dose adjustment, or standalone prescription management, a 15-minute HRT Prescription Management phone call ($125) is available on most second and fourth Tuesdays of each month.

The cancellation policy is a minimum of 48 hours before your scheduled appointment. For example, a Monday appointment would need to be canceled on the prior Thursday. For a first-time cancellation with less than 48 hours notice, clients will be charged a $50 fee. For any other cancellations after that, with less than a 48 hour notice, clients will be charged the full appointment rate.

This rate reflects not only your face-to-face time but also the behind-the-scenes review, preparation, lab interpretation, research, and follow-up completed to meet your individualized needs.
Follow-Up or Annual Wellness Appointment (60 min) - $200
For reviewing lab results, specialty test results, and refining or updating wellness protocols.
Follow-Up Appointment (30 min) - $125
For focused check-ins, protocol adjustments, and general support.
HRT Initiation Add-On - $95
A one-time fee billed separately after your initial or follow-up visit when you and your provider decide together to move forward with hormone therapy for the first time. Hormone therapy prescribing involves costs beyond a standard clinical visit, including a supervising physician collaborative fee, an e-prescribing platform, and specialized malpractice coverage. This fee reflects both the clinical service and the cost of maintaining a safe, compliant prescribing practice.
HRT Prescription Management Visit (15 min phone call) - $125
Required for all prescription refills, dose adjustments, and formulation changes. All prescription changes require a documented clinical visit as a legal and clinical standard of care. Available most second and fourth Tuesdays of each month.
