You will leave equipped with the knowledge and tools to understand your own hormonal picture and advocate for yourself with your primary care provider or prescriber. Geography does not have to stand between you and real answers.

If you are new to NaturaVita, we start with a full 90-minute intake. We go deep into your health history, your symptoms, your labs, your life. If hormone therapy is appropriate, we can begin the conversation and in many cases initiate prescribing at the same visit.

Hormone therapy at NaturaVita is never one-size-fits-all. The formulation, delivery route, dose, and timing are chosen based on your specific clinical presentation, health history, goals, and available labs. We work with a full range of bioidentical and pharmaceutical hormone preparations including estradiol, progesterone, testosterone, DHEA, and vaginal formulations across transdermal, oral, vaginal, and sublingual routes.

Hormones shift. Life shifts. Your protocol shifts with you. Brief prescription management phone appointments are available for refills, dose adjustments, and formulation changes on an ongoing basis. Every prescription change is documented with a clinical assessment because that is both the legal standard and the right approach to care.


How It Works

Our Prescribing Partnership

Hormone therapy prescribing at NaturaVita Wellness operates under a formal Collaborative Practice Agreement with Dr. Timothy Paik-Nicely, MD of With Open Arms Healthcare.

Dr. Paik-Nicely brings nearly 50 years of experience in family medicine and women's health to this arrangement. Many of his clients have overlapped with NaturaVita over the years. During my time at Moonstone Midwives Birth Center, he was often the physician on the other end when care needed to move to a hospital setting. We built trust and respect over a long shared history of caring for this community. Having him as the supervising physician in this prescribing partnership is both a legal arrangement and a genuinely well-matched collaboration between two providers who share a commitment to doing right by the people in their care.

What the Current Evidence Actually Says

For over two decades, women were told to fear hormone therapy based largely on a single 2002 study that has since been substantially reanalyzed, reinterpreted, and challenged by a far stronger body of evidence. Here is what the science now shows:


  • Your brain runs on estrogen. Groundbreaking brain imaging research has revealed that as estrogen declines during perimenopause and menopause, estrogen receptor density in brain cells actually increases, meaning the brain is actively seeking the estrogen it is no longer receiving. This elevated receptor density is directly associated with cognitive symptoms, mood changes, memory disruption, and brain fog. The brain changes that occur during the menopause transition involve shifts in structure, regional connectivity, and energy metabolism. Estrogen-based hormone therapy is now being actively investigated as a potential strategy for reducing the risk of Alzheimer's disease, a condition women develop at twice the rate of men. The hormonal connection is not incidental. It is central.

  • Your heart, bones, and muscles are also affected. When initiated within 10 years of menopause, hormone therapy is associated with a 30 to 50 percent reduction in cardiovascular disease risk and meaningfully lower overall mortality. Women on hormone therapy have cholesterol levels averaging 12 percent lower than those not on it. HRT produces greater improvements in bone mineral density than exercise therapy alone and significantly reduces fracture risk at the hip and spine. Twenty-five percent of elderly women who sustain a hip fracture die within a year. Bone health is not a cosmetic concern. It is a longevity one. Hormone therapy also reduces the risk of sarcopenia, the progressive loss of muscle mass that accelerates sharply after menopause.

  • Delivery route matters enormously. Transdermal estradiol and vaginal estrogen bypass first-pass liver metabolism entirely, which eliminates the clot and stroke risk associated with oral estrogen. Micronized progesterone carries a far more favorable safety profile than synthetic progestins. Vaginal estrogen has minimal systemic absorption and is considered appropriate across essentially all health histories. These are not minor distinctions. They are the difference between outdated fear and current, individualized, evidence-based care.

  • The breast cancer conversation has changed. Estrogen taken alone, without synthetic progestins, is associated with a 29 percent reduction in breast cancer incidence in some studies. Transdermal delivery and micronized progesterone carry a significantly more favorable safety profile than the synthetic hormones used in the original 2002 research. In November 2025, the FDA removed the decades-old black box warning from HRT products, formally acknowledging that the risk-benefit profile had been significantly misrepresented for an entire generation of women.

  • There is no mandatory age to stop. Current clinical consensus is clear: there is no arbitrary age cutoff for hormone therapy. A 2024 study examining over 10 million Medicare beneficiaries over age 65 found meaningful ongoing benefits of estrogen therapy in this population. Modern care uses individual risk assessment, not one-size-fits-all rules.

  • Testosterone belongs in this conversation. At physiologic doses, testosterone improves libido, energy, mood, muscle mass, and cognitive function in women. Current evidence also suggests a potential protective benefit against breast cancer at physiologic doses. Testosterone deficiency in women is broadly undertreated and under-discussed. It does not have to be.

Hormone therapy prescribing involves costs beyond a standard clinical visit, including a supervising physician collaborative fee, an e-prescribing platform, and specialized malpractice coverage. The fees below reflect both the clinical service and the cost of maintaining a safe, compliant prescribing practice.

Required for all prescription refills, dose adjustments, and formulation changes. All prescription changes require a documented clinical visit as a legal and clinical standard of care.

HRT Initiation Add-On: $95

A one-time fee billed separately after your initial or follow-up visit when you and your provider decide together to move forward with hormone therapy for the first time. Covers your first prescription, pharmacy coordination, and clinical setup.
